Important Health Reform Rights - Dependants Under age 26 Information InformationAugust 1, 2010
Important Health Reform Rights
Opportunity to enroll or re-enroll dependents under the age of 26

If you have a dependent whose coverage ended, or who was denied coverage (or was not eligible for coverage), because coverage for dependent children under the plan previously ended before they were age 26, they are eligible to enroll or reenroll in your employer’s health plan with Blue Shield.

You may request enrollment for such children who are under age 26 during your enrollment period. Enrollment will be effective as of the first day of your employer’s first plan year beginning on or after September 23, 2010, even if that results in retroactive enrollment.

For more information contact the CCPOA Benefit Trust Fund at (800) IN UNIT 6 or call Blue Shield Member Services at (800) 257-6213.

A few good reasons to go with CCPOA Medical PLAN:

1. Great Features. That’s really what matters, right? In Northern California we are the only HMO plan that offers the full Sutter Health Network of more than two dozen hospitals and 3,000+ physicians. In Southern California we are the lowest rates around. As always, the Trust worked hard to keep prices down. Many of the most common reasons to see the doctor have no co-pay for members. The services people use most, like well-baby care, an X-ray, or immunizations can be zero out of pocket to you. Almost every other service has a low co-payment of only $15. Why pay extra when you don’t need to? Keep more of your paycheck each month.

2. It's a custom fit. The CCPOA Medical Plan is designed for Correctional Officers by Correctional Officers. Our Trustees are elected by CCPOA members just like you. The way that our members use health services are factored in from the start. We have the ONLY plan that offers network chiropractic coverage. The CCPOA Medical Plan fits your lifestyle better than just about anything else out there.

3. Virtually no deductibles. Most services have no deductible to meet before you are fully covered. Services requiring a co-pay are almost always only $15.
And don’t forget that the CCPOA Medical Plan is administered by Blue Shield of California, which has one of the largest provider networks around.
